Ik zal tevens vragen stellen indien mij iets niet duidelijk is.Ik ben mij er bewust van dat er mogelijke risico’s zijn met betrekking tot het krijgen van een tatoeage; bloedvergiftiging, lokale zwelling en trauma rondom het tattoo gebied, lokale infectie, lijn/inkt uitlopers, mogelijke littekens en allergische reacties.Ik ben niet onder invloed van alcohol of drugs en ik sta toe dat ik zonder dwang word getatoeëerd.Ik heb de tatoeëerder geïnformeerd wanneer ik een van de volgende symptomen heb: covid-19/corona, diabetes, epilepsie, een hartaandoening, hiv-infectie, hepatitis A, B of C, gebruik van bloedverdunners, zwangerschap of een huidaandoening heb welke nadelig kan zijn voor het aanbrengen van de tatoeage of het genezen ervan.Ik heb geen geestelijke stoornis welke effect kan hebben op mijn beoordelingsvermogen.Noch de Artiest danwel Blackbird Tattoo Studio is verantwoordelijk voor de betekenis van het symbool of de tekst welke ik aan hen heb gegeven of welke ik gekozen heb van een flash (ontwerp) sheet. Ik als klant ben tevens zelf eindverantwoordelijk voor de keuze van het uiteindelijk gekozen ontwerp, de plaatsing hiervan en het formaat.Een tatoeage is een permanente verandering aan mijn uiterlijk en kan enkel verwijderd worden door laserbehandeling of via chirurgische methoden, welke misvormend en/of duur kan zijn, en waarschijnlijk niet het resultaat zal hebben van herstel van mijn huid zoals deze was voor de tatoeage sessie. I will ask if there is an instruction I do not fully understand.I am aware of the potential risks from tattooing; blood poisoning (septicemia), localized swelling and trauma around the tattooing area, localized infection, line blowouts, potential scarring and allergic reactions.I am not under the influence of alcohol or drugs, and I am voluntarily submitting to be tattooed by the Artist without duress or coercion.I have informed the tattooist if I have any of the following conditions: covid-19/corona, diabetes, epilepsy, hemophilia, a heart condition, HIV infection, Hepatitis A, B & C or any other medical or skin condition that may interfere with the application or healing of the tattoo. If I take blood thinning medication. Or if I am pregnant.I do not have a mental impairment that may affect my judgement in getting the tattoo.Neither the Artist nor Blackbird Tattoo Studio is responsible for the meaning or spelling of the symbol or text that I have provided to them or chosen from the flash (design) sheets. I also agree that I am solely responsible for approving the design, it’s placement and size anywhere on my body, the artist merely advices me on the proper application of the design I chose to have as a tattoo.A tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which can be disfiguring and/or costly and which in all likelihood will not result in the restoration of my skin to its exact appearance before being tattooed.
